オンダリフトとエムフェイスはどんな機器?(EBDの進化Ⅶ)

チタニウムリフト(SOPRANO TITANUM)とは?
Alma KoreaのSOPRANO TITANUMとDEKA KoreaのONDAは、「reboot(再起動)」や「rebirth(再生)」という言葉がよく合う機器です。脱毛機だったSOPRANO TITANUMは、Alma Koreaと韓国の医師たちによって新たな施術へと生まれ変わり、世界に広がっています。755㎚、810㎚、1064㎚を組み合わせて照射するこの機器は、発色団を直接狙うわけではありません。選択的光熱融解理論を拡張した考え方に基づき、熱拡散によって毛の幹細胞に働きかけます。
SOPRANO TITANUMのリフトアップ効果に懐疑的な見方からは、一般的な1064㎚・300msecのジェネシスレーザーやデュアルトーニングと変わらない、という意見もありました。毛に関連する周辺組織への作用がまだ明確に解明されていないことには、それなりの理由があると考える医師もいます。
以前、ロングパルスNd:YAGレーザーとRFを、顔の左右で比較した研究がありました。Mark B. Taylorらの「Split-face Comparison of Radiofrequency Versus Long-pulsed Nd:YAG Treatment of Facial Laxity」と、Douglas J. Keyらの「Single-treatment Skin Tightening by Radiofrequency and Long-pulsed, 1064㎚ Nd:YAG Laser Compared」では、ロングパルスNd:YAGもRFに劣らない効果を示しました。しかしHIFU登場後もモノポーラRFは使われ続けた一方、ロングパルスNd:YAGは広く普及しませんでした。痛みが強く、わずかな不注意で3度熱傷を起こす可能性があったためです。SOPRANO TITANIUMは、脱毛効果と安全性を高めるために、755㎚、810㎚、1064㎚の同時照射と、強力な接触冷却システムを備えています。これが、痛みを伴わずに良好な結果を得られた理由と考えられます。

オンダリフト(ONDA)の仕組み
DEKA KoreaのONDAは、ボディ用の機器として使われた後、顔にも応用され、世界へ広まっています。従来のRF機器が1・2・4・6.78・40.68㎒などの周波数を使うのに対し、ONDAは2.45㎓を使用します。極性分子である水が回転して摩擦熱を生む、という組織加熱の原理からみると、ONDAは発熱効率に優れています。また、周波数が高くなると、通電による伝導加熱よりも、誘電的な振る舞いによる電磁場、組織間の導電率や誘電率の影響が大きくなります。その結果、皮膚よりも脂肪層に多くのエネルギーが届きます。モノポーラRFはバイポーラRFより深くまで届きますが、脂肪層に伝わる量は皮膚より少なく、脂肪層では線維性隔壁に沿ってのみ流れる傾向があります。一方、ONDAは皮膚の層より、脂肪層全体に多くのエネルギーを届ける傾向があります。

ONDAのマイクロ波は、各国で特許を取得したリターン電極技術によって、目的の深さにエネルギーを集中させます。初めはボディ用として、たるんだ肌のハリやセルライトを改善し、脂肪を減らす機器として紹介され、米国やヨーロッパで広く使われました。リターン電極を応用した3㎜ハンドピースの開発後、韓国では食品医薬品安全処の承認に時間がかかり、導入が1年以上遅れました。しかし、顔に使うと、お腹や太もものセルライト治療に似た効果が得られたため、導入直後から大きな人気を集めました。

RedTouchレーザーは、どうコラーゲンに働きかける?
DEKA KoreaのRedTouchは675㎚のレーザーで、コラーゲンへの選択性を大きな特徴としています。従来の光による若返り機器は、水への吸収の度合いに応じてそれぞれ異なる方法で、目標とするコラーゲンに熱を届けていました。拡張された選択的光熱融解理論を用いるこれらの機器に対し、675㎚のRedTouchはコラーゲンの選択的光熱融解を直接目指す機器といえます。この波長はメラニンにも比較的よく吸収されるため、初期の論文は肝斑治療を扱っていました(「A new 675㎚ Laser Dvice in the Treatment of Melasma: Results of a Prospective Observational study」)。その後、コラーゲンに対する作用が注目され、顔の老化、ニキビ跡、肉割れなど、さまざまな適応への使用が確認されました。コラーゲンを直接対象にする方法は、水を発色団とする従来の方法に比べ、ダウンタイムや痛みが少ないという特徴があります。乳頭真皮にエネルギーが集中し、より新しいコラーゲンが多くつくられることも期待できます。臨床では、痛みやダウンタイムの負担に比べて変化が早く、肝斑、赤み、毛穴、肌の明るさやキメに加え、顔全体の輪郭やしわにも改善がみられます。顔の赤みへの効果は、基底膜に近い血管叢の周囲のECMが改善するためと考えています。

エムフェイス(EMFACE)は、どう筋肉に働きかける?
BTL KoreaのEMFACEは、顔の若返りを目的に、HIFEM(High-intensity Focused Electromagnetic)刺激とRFを組み合わせる施術です。HIFEMで額の前頭筋と、顔の中央の大頬骨筋・小頬骨筋・笑筋に最大随意収縮を超える収縮を起こし、顔面組織の位置の回復と再構築を促します。作用機序はまだ完全には解明されていませんが、筋肉につながるさまざまな顔面組織の位置や構造の変化は、臨床でもある程度確認されています。
